CareCollab is committed to protecting the privacy and security of your personal health information. Call Recording Notice
To enhance the quality of our services, calls may be recorded for training and informational purposes. If you prefer not to have your calls recorded, please notify us at the beginning of your call.

CareCollab provides text messaging services for appointment reminders, medication updates, application assistance and completion, routine check-ins, and other relevant updates. By opting into these services, you agree to the following terms:
- Messaging Frequency: Messaging frequency may vary depending on the type of notifications and your interactions with our services.
- Message and Data Rates: Message and data rates may apply. Please check with your mobile service provider for details.
- Opt-Out Instructions: To stop receiving messages, text STOP at any time. This will unsubscribe you from further communications.

Our Responsibilities
- CareCollab is required by law to maintain the privacy and security of your protected health information.:
- CareCollab will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
- CareCollab must follow the duties and privacy practices described in this notice and give you a copy of it.
- CareCollab will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time. Let us know in writing if you change your mind.
Changes to the Terms of This Notice
We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office, and on our web site.
